You deworm. You rotate. You dose on schedule. And then you pull a fecal egg count and the number comes back higher than you expected — again. If you’ve been in this loop with a horse, you already know that a tube of paste and a calendar reminder isn’t going to get you out of it.
High parasite burden horses exist on every type of farm, in every management setup. They’re not a failure on your part. But managing them well requires understanding why some horses carry heavier loads than others, what you’re actually treating, and how to build a program that does more than spin the resistance wheel faster.
Fecal Egg Counts Are Not Optional If You’re Doing This Right
This is where I’ll lose some people, and I’m okay with that.
If you’re deworming based on a schedule instead of fecal egg counts (FECs), you’re guessing. You might be over-treating a horse who doesn’t need it and under-treating the one who does. Anthelmintic resistance — the real, documented kind, not a theoretical concern — is largely a product of overuse. Strongyle resistance to benzimidazoles (fenbendazole, oxibendazole) is already widespread. Cyathostome resistance to pyrantel is documented. We are borrowing time.
A baseline FEC before treatment and a fecal egg count reduction test (FECRT) two weeks after treatment tells you whether your dewormer actually worked. If your vet hasn’t brought this up, bring it up yourself.
What the numbers mean in practical terms:
- Under 200 EPG (eggs per gram): low shedder — typically treat 1-2x per year
- 200–500 EPG: moderate shedder — more frequent monitoring warranted
- Over 500 EPG: high shedder — this is the horse we’re talking about today
High shedders make up roughly 20% of a herd but produce 80% of the environmental contamination. If you have one of these horses, pasture management is not optional.
Why Some Horses Are Chronic High Shedders
Parasite burden is partially genetic, partially immune. A horse with a healthy, trained immune response will typically suppress strongyle populations on his own. A horse whose immune system is compromised — by stress, chronic inflammation, poor nutrition, age, or concurrent illness — will not.
Common reasons a horse stays a high shedder:
- Stress load is too high. Horses in heavy training, horses who are chronically anxious, horses who aren’t sleeping well — their cortisol is elevated, their immune function is suppressed. This isn’t a deworming problem. It’s a whole-horse problem.
- Gut health is compromised. A healthy hindgut microbiome contributes to parasite resistance. A horse on repeated courses of dewormers, or one who’s had multiple rounds of antibiotics, or one with chronic loose manure — that gut environment isn’t doing the work it should.
- Nutrition is inadequate. Specifically protein, zinc, and vitamin E. A horse who’s marginally nutritious doesn’t have the raw materials for a robust immune response. Fancy supplements don’t fix a base diet that’s missing the mark.
- Age. Horses over 20 and foals under 18 months are at higher risk. Their immune systems aren’t doing the same job.
- Concurrent PPID (Cushing’s disease). If your older horse keeps coming up as a high shedder and you haven’t tested for PPID, do it. The immune dysregulation from untreated Cushing’s makes parasite management significantly harder.
The Treatment Protocol for High Burden Horses
Work with your vet on this. I mean it. What follows is framework, not a prescription.
Step one: identify what you’re dealing with. A regular FEC tells you egg burden. A larval development assay or a PCR-based test can identify specific parasite species and resistance patterns. If you’re seeing high counts repeatedly, you want to know if you’re looking at small strongyles (cyathostomes), large strongyles (rare now but serious), tapeworms, or pinworms. Each responds to different drug classes.
Step two: treat strategically, not reflexively. For confirmed high shedders, your vet may recommend a larvicidal dose of fenbendazole (5-day Power Pac protocol) to address encysted cyathostome larvae — the ones living in the gut wall that a single dose of anything won’t touch. This is typically used once a year, in fall or early winter, and only when appropriate. Not every horse needs it.
Step three: the FECRT. Pull another fecal 14 days post-treatment. If egg counts haven’t dropped by at least 95%, you have a resistance problem that needs to be addressed, not ignored.
Step four: interval monitoring. High shedders should be tested every 8–12 weeks until you have a clear picture of their individual pattern. Some horses are high shedders in spring and fall and much lower in between. Some are consistently high year-round. The data shapes the protocol.
Pasture Management Is Half the Battle
A deworming program without pasture management is like bailing a sinking boat without plugging the hole.
Strongyle larvae survive on grass. They survive especially well in warm, moist conditions — which means your summer pastures in the southeast are actively working against you right now. The life cycle from egg to infective larvae in summer heat can be as short as three to five days.
What actually helps:
- Pick up manure. Every two to three days from small paddocks or sacrifice lots. Once a week minimum from larger pastures. This is the single highest-impact management change most people aren’t doing consistently.
- Avoid overgrazing. Horses graze close to the ground. Infective larvae concentrate in the first few inches of grass. Rested pastures with longer grass reduce intake.
- Rotate pastures. Ideally, resting a pasture for six to eight weeks between grazing cycles gives time for larval die-off.
- Harrow strategically. Harrowing spreads manure and exposes larvae to UV light and drying. It’s most effective in dry, sunny weather. In humid summer conditions, harrowing can actually spread larvae — so time it right.
- Quarantine and test new horses. Every new arrival should be FEC’d before going onto your shared pastures.
The Stiff or Unthrifty High-Burden Horse
Here’s a clinical reality that sometimes gets missed: a horse with a heavy cyathostome burden — particularly encysted larvae — can present as vague, hard to put weight on, poor hindgut motility, intermittent soft manure, or general malaise. The symptoms can mimic ulcers, low-grade colic, or musculoskeletal stiffness.
If you have a horse who’s been moving short behind, struggling with forward, or just not performing like himself — and you haven’t pulled a recent FEC — do it before you book another bodywork appointment. Parasites can create real, physical discomfort that shows up in the saddle before it shows up anywhere else.

Supporting the High-Burden Horse Between Treatments
Deworming is crisis management. What you do between doses matters more over time.
- Hindgut support: Probiotics and prebiotics won’t replace a healthy diet, but they can help rebuild after treatment. Look for products with documented strains and actual CFU counts — you can find what I actually use reviewed over on the blog.
- Targeted nutrition: Get a hay analysis done. If your forage is low in protein or selenium, no supplement stack will make up for it.
- Stress reduction: This one is underrated. A horse who’s anxious, reactive, or worked in a chronic state of sympathetic activation will have a harder time regulating immune function. How you ride and how you handle matters here too.
The Bottom Line
A horse with a high parasite burden isn’t a problem you deworm once and move past. He’s a horse who needs a real program — strategic treatment guided by actual lab data, meaningful pasture management, and underlying health support that gives his immune system a fighting chance.
